If your organization has spaces at multiple addresses, please fill out this form for each location and note the difference in the "Organization Name" field. (i.e. Columbus Metropolitan Library - Parsons Branch)
If your organization has multiple spaces at the same address, please fill out this form as if it were one space (i.e. 3000 sq. ft), and note the number of rooms in the final "Tell us more about your space" question.
