Supervision
Reports to the Get Connected Program Manager
Schedule
Work hours are generally 8:30 am – 5:00 pm Monday through Friday, flexible based on work demands and in consultation with a supervisor, 40 hours per week. This is a full-time, exempt position. Some evenings and weekends are required for support group coverage.
Program Responsibilities
• Coordinate the daily operational elements of and participant engagement with the Get Connected (GC) Program
o Coordinate the Get Connected Care Navigation phone line
o Answer incoming calls to the Get Connected Care Navigation line and the main phone line
o Manage voicemails and text messages from the main agency phone line and the Get Connected Care Navigation line – check messages and add new callers to the Get Connected database promptly, alert staff to new voicemails directly for them
o Coordinate all incoming referrals to the GC Program
o Check Sfax daily and enter new referrals into the program database promptly
o Maintain and update the Sfax provider contact list
o Monitor the GC Program email inbox and respond promptly to messages, direct messages to appropriate staff, and add new emailed referrals to the program database promptly
o Assist GC Program Manager with updating and maintaining the GC Program provider directory
• As a member of the GC Program Team, provide daily core program services
o Assist and advocate for program participants by providing the following GC Program Care Navigation services:
o Timely information and referral services for program participants who are seeking behavioral health services in Central Ohio
o Provide system navigation and community resource linkage to program participants who have experienced difficulty connecting to care due to psychosocial or systemic barriers
o Coordinate the Get Connected Program participant follow-up process
o Complete all scheduled follow-up outreach to program participants, referral sources, and community providers to verify successful linkage
o Send out electronic surveys to completed program participants via email or SMS
o Provide facilitation coverage for support groups, as needed
• Balance individual daily direct service workload with other job responsibilities in accordance with program policies and procedures
• Consistently participate in agency and program events, groups, activities, and work collaboratively with agency staff
• Other duties, within reason and scope of the position’s primary duties, may be assigned after consultation between the employee and supervisor
Salary:
$41,000 – $47,000 per year. Salary commensurate with experience and other qualifications.
Required Qualifications:
• Education in social services or related fields
• Experience working with diverse communities
• Strong mediation and de-escalation skills
• Strong verbal, written, and organizational skills
• Comfortable working with individuals living with mental health and substance use disorders
• Experience with Microsoft Office 365 applications and macOS
• Valid driver’s license or means of transportation
Preferred Qualifications:
• At least two years’ experience working in the social services
• At least two years’ experience working in the mental health/SUD system in central Ohio
• Experience with web-based databases
