Freedom a la Cart is an innovative, nonprofit social enterprise and workforce development program born to help survivors of human trafficking build a new life of freedom and self sufficiency. What started as a single food cart is now a bustling, full-service catering company, grown from the hearts and hard work of the women who sustain us. Through offering wrap around supportive services, practical life skills, paid workforce training and one-on-one case management, Freedom is a place where survivors can heal, learn and grow on their journey toward personal and economic self-sufficiency.
We are looking for a proactive, determined, and positive Case Manager capable of managing the various needs and resources of our growing sisterhood of survivors. This role will require a flexible work schedule and “can do” attitude to encourage the growth and restoration of those served.
CASE MANAGER (FT, 30+ hours per week)
The overall vision of the case manager position is to work one-on-one with survivors to help them achieve personal goals, stability, and self-sufficiency.
We are seeking someone with unique interpersonal skills capable of interacting effectively with survivors to provide for the needs and goals of our women on an individual basis.
Overview of Responsibilities:
- Work effectively with other team members to fully understand the needs of each client by scrutinizing their background history, understanding their current needs, and arranging for their well-being.
- Coordinate with various service providers to ensure the fulfillment of client needs.
- Ensure proper follow ups, and have face-to-face interactions with the client, and other parties involved. Review previous goals and assess if there are new needs.
- Be well-versed with various parameters that would make the activities and functions of a case manager most effective. This includes actively seeking resource partners; obtaining adequate knowledge about resources and the appropriate paperwork required to access those benefits; and the ability to adjust all these activities according to the needs of the client.
- Maintain confidentiality regarding the information that the client is likely to share with the case manager.
- Keep proper documentation of all work. This involves records of the client’s information, the meetings held with the clients and service providers, details of the referrals that were issued, the cost of the services, the plans and goals that were outlined to ensure a positive outcome, and so on.
- Ensure workforce safety by performing regularly scheduled random drug screens and consequently, address any relapse issues by developing individual relapse plans.
- As a mandated reporter, report all suspected physical or sexual abuse of minor/elderly, homicidal or suicidal intentions.
- Oversee and coordinate jail transportation from jail to treatment centers.
- Work with the Director of Programs to plan and coordinate data collection and evaluation plans to meet programmatic goals and targets.
- Create a positive work environment that fosters teamwork, personal growth, and confidence.
- Other duties as assigned.
Qualifications:
- Associates degree or higher in Social Work or a related field
- Adaptable people person with strong social/interpersonal skills and ability to communicate well with diverse audiences
- Knowledge of creating and utilizing Google applications Docs, Sheets, Gmail and Slides
- Experience in working with vulnerable trauma survivors strongly preferred
- Conscious of and sensitive to the diversity within Freedom a la Cart and able to interact professionally with this diverse population of people from many different backgrounds
- Valid driver’s license with less than six points, personal vehicle and proof of insurance.
